How to obtain my medical records
For most patients, a copy of their operative reports, breast implant registration numbers and/or photos are all that will be needed for your new surgeon. If you would like a complete copy of your entire medical chart, an administrative fee of $125-$200 may be applied based on the size of the medical chart and number of pages processed for pick up.
All patient charts will be transferred to MedRecords for secure and confidential storage long term management. After the office closure on October 30th 2026, you can contact MedRecords directly for your medical chart.
