Fat Grafting Breast Augmentation
Some of the women considering breast augmentation at YES Cosmetic Surgery Centre are interested in enhancing the size of their breasts without implants. Transferring unwanted fat from another area of the body to the breasts—a technique called fat grafting—has become a common option for breast augmentation patients at our practice. Fat grafting is not a new technique for improving the contours of a patient’s face, breast, or body. Dr. Mosher has been performing the procedure on selected patients for more than 20 years.
Recent technique improvements, however, have resulted in more predictable and safer outcomes. Transferring large volumes of fat isn’t predictable, so patients who want a significant increase in the size of their breasts are typically not good candidates for breast fat grafting. Women seeking an increase of 1 to 2 bra cup sizes, or less 200 cc of additional volume are often able to achieve this lasting enhancement using their own fat. A secondary benefit of this technique is improving the shape in areas of the body where Dr. Mosher typically harvests the fat, such as the thighs, abdomen, or flanks. Experience and technique are essential components of more successful and safer fat augmentation procedures.

Good Candidates for Breast Augmentation
What makes someone a good candidate for breast augmentation? Firstly, you should be in good overall health and have made the choice to get breast implants because it’s what you want, rather than because your spouse or significant other wants you to have larger breasts. Secondly, patients must have realistic expectations about what breast enhancement surgery can accomplish for their unique anatomy. During the consultation, Dr. Mosher uses his expertise to recommend the type and size of implants as well as the surgical techniques that will be best-suited to achieve the aesthetic goals you describe.
Finally, breast augmentation patients should be healthy and should not be pregnant or breastfeeding at the time of the procedure.

Your Breast Augmentation Procedure
Breast augmentation is performed as an outpatient general anesthetic procedure at our modern and fully accredited YES Surgery Centre, located on the same floor as our office. Dr. Mosher has refined the breast augmentation surgery to reduce the duration of the procedure, reduce the risks of complications and to reduce the rates of revisional surgery over the life span of your breast implants. He and his hand-selected team of professionals have completed more than 5,000 breast augmentation procedures with a less than 1% risk of early complications that would require more surgery or medical treatment. Our patients benefit from our proprietary Recipe for Recovery™ which reduces their post-operative pain and speeds their return to regular daily activities.
Once you’ve decided to have breast augmentation, there are many options available for tailoring your procedure.
Dr. Mosher has extensive experience with the most recent innovations in breast implants available in the Vancouver area. He offers a full range of options, including MemoryGel® implants. Dr. Mosher also has extensive experience with saline filled breast implants. Your choices include:
- Saline implants are filled with a medical grade saltwater solution which is harmlessly absorbed by the body if leakage or rupture occurs. Since saline implants are filled after placement, they can be inserted through more incision locations and their final volume adjusted in the operating room to achieve optimal results. Despite these features, these have become less popular choices in recent years for a variety of reasons. Generally, when compared to silicone gel implants, patients find that saline implants feel and move less like natural breast tissue. They have also been associated with a higher rate of revisional surgery attributable to factors such as more frequent problems of implant shell ripples and wrinkles.
- Silicone gel filled implants have rapidly gained in popularity among breast augmentation patients since becoming approved by Health Canada in 2001.
Patients considering breast augmentation surgery will also be educated about available implant shapes and profiles. Anatomical or contoured implant shapes rarely recommended but are available for particularly challenging breast shape asymmetries. The most commonly recommended shapes are round implants. When appropriately selected to match the anatomical features of each patient, a round implant can achieve natural looking results with more filling of the central and upper breast contours. The implant profile selection permits a more customized “fit” for each patient.
Dr. Mosher offers VECTRA® 3-D imaging to help patients better understand their unique anatomy and can preview the final outcome associated with implant recommendations. This innovative and scientifically validated system, captures a 3D photo of your body and permits simulations of your breast augmentation surgery outcome. Dr. Mosher considers different incision options for each patient through which the implant is inserted.
- Inframammary incision: a 3–5 cm long incision placed along the natural crease under the breast
- Periareolar incision: a 4–6 cm long incision along the bottom edge of the areola (the dark skin surrounding the nipple).
Dr. Mosher explains the benefits of each option and he will help you understand how your body type and the type of implant being used influence which type of incision would be most effective. After making the incision, he inserts the implant into the breast pocket either beneath the breast tissue (subglandular), or under both the breast tissue and the chest muscles (subpectoral). One method isn’t necessarily better than the other, but your best option will depend on a variety of factors, which Dr. Mosher can help you determine.
Dr. Mosher uses careful surgical techniques and instruments that minimize tissue trauma, resulting in less bruising and bleeding for a much more comfortable and rapid recovery.

Recovery and Results
Dr. Mosher’s high level of experience and surgical skill allows him to minimize stretching and trauma to surrounding tissues, which greatly reduces post-operative discomfort. Some swelling and discomfort are to be expected, and these are managed with oral medications, stretching exercises and cool compresses. You will wear a snug-fitting sports bra for approximately 2 weeks to provide support and to help with healing.
Our unique program for rapid recovery following breast augmentation allows many of our patients to go out for dinner in the evening following their surgery. Most patients return to light activity and work within a few days of their surgery, but strenuous exercise or weight lifting must be avoided for at least 4 weeks. Your breasts will look noticeably larger right away, but it will take up to 3 months for the tissues to relax and for the breasts to “settle” into a more natural shape.
